Holistic Practitioner Licence Application Support
Eligibility Requirements
Applicants must meet the following conditions:
- Be at least 18 years old
- Provide a Criminal Record and Judicial Matters Check (level 2)
issued by a Canadian Police Service within 280 days prior to the application. - Provide a photo of the applicant that complies with the Licence Application Photo Requirements
- Provide an original membership letter or certificate from an approved Holistic Association.
- Provide two pieces of valid identification, including:
• One photo ID
• One proof of legal work status in Canada

Licence Renewal Information
Your licence will expire annually on the anniversary of the date that the licence was first issued.
If you fail to renew your licence according to the bylaw, your licence will be cancelled and you will have to reapply.
